Special Form

I-9 Form

The I-9 Form is a government form used to verify your eligibility for employment. Every new employee must complete this form in person with your supervisor. (See your supervisor for assistance.) As long as you are continually employed by the institution, you are not required to complete this form a second time.

Change Forms

When you are initially hired, you will complete your forms through the online on-boarding system. The forms below are provided if you need to make changes after you have been hired.

W-4 Form

The Form W-4 if you would like to change your federal income tax deduction.

Residency Certification Form

This form is used to determine your local income tax deduction, which is based on where you live. Any fields that you do not know you may leave blank and the Payroll Office will fill them in. You can click here to find your PSD Code.

Direct Deposit Authorization Form

We encourage all employees to receive their pay through a direct deposit into their bank account. The account may either be a checking account or a savings account. Use this form to let us know your bank and account number. You may click here for a fillable Microsoft Word version of the document.

NOTE: The Payroll Office requries that you deliver the direct deposit form to the payroll office in person. Bring suitable ID to confirm your identity, such as your university ID or a driver's license. Contact the Payroll Office with questions.
