Official academic transcripts are sent upon receipt of the student's written request.

Enrollment verification can be obtained for insurance, loans, scholarships, etc.

Replacement credential(s) and diploma cover may be replaced upon request.

Notary and Pennsylvania State endorsement of student's diploma or transcript for international service or employment.

Request your academic record be re-evaluated for completion of degree requirements.

Former ADP students wishing to return and complete additional coursework must submit this form.
